The second part is a brief questionnaire so we know what playlists the author thinks their map fits in. Note: Players who want a playlist devoted around their map are going to be disappointed. In fact, players whose maps cater to a very specific gametype may find it more difficult to get their playlist accepted than someone who makes maps for the existing playlists.
The last part is video, Halo 3 film or youtube clip or gamevee or whatever, of the map being played. At least a couple will be expected. At least two films showing full games being played, from multiple players points of view.
Screenshots will be expected as well in the Halo 3 file forum thread the map is posted too, but not required.
Like I've said before, it won't be easy, and a lot of people are going to complain, but so be it.

To answer some of the other questions in this thread:
We will expect people to post publish their maps to the Files forums here on Bungie.net. We're not going to explore other websites to hunt down map variants, nor are we going to "give" map selection duty to a community, like we've done with Bungie Favorites.
Certain playlists will not be open for submissions. MLG and Grifball, for example.
Geometry interlocking is fine. However, if the Z-fighting looks bad (that is, when you've got two pieces of geometry overlaying each other and are "fighting" to be the part that's drawn first), or if the interlocking just looks bad in general, your map might not be accepted.
Sculptures, jumping puzzles, and gimmicks will not be considered for Matchmaking.
The flame effect used in flame grenades and the flamethrower, as well as the fuel rod projectile are complicated effects that affect performance and can cause problems on local area network as well as LAN. Neither the flame grenade or the fuel rod gun will be allowed in Matchmaking maps, and their presence will result in immediate dismissal.
I won't have time to individually write a critique for every map submitted. Submitters should not expect a personalized post in their forum by a Bungie employee to tell them why their map isn't in matchmaking.
