No it doesn't have to be a picture of yourself. It's anything related to you, your username or gamertag or something. For example, my name here is Pigglez... I could do a sig of pigs or something like that.

Yea, I mean my username is taken from Oblivion and my Gamertag is Apples R Us, not that difficult. You should know WHY you picked your username/gamertag, so I don't see this as too difficult a task.
